On-Demand Webinar: Putting Together the Discharge Plan

Recorded On: 11/02/2023

When it comes to discharge planning, most long-term care professionals can recite some of the pieces—discharge planning starts on admission, requires the entire interdisciplinary team, and doesn’t stop on discharge. But how does a team use all these pieces to meet both the federal regulations and the RAI User’s Manual requirements for discharge planning?

During this 60-minute webinar, AAPACN Curriculum Development Specialists, Denise Winzeler, RN, BSN, LNHA, DNS-MT, QCP-MT, and Jessie McGill, RN, BSN, RAC-MT, RAC-MTA, will provide a realistic approach to putting together a discharge plan that meets all of the requirements and helps to achieve successful discharge goals.

After participating in this session, learners will be able to:

  • Recognize the federal requirements for F-tag F660, Discharge Planning Process
  • Understand how MDS data is used to develop a comprehensive care plan that addresses the discharge plan
  • Identify facility systems and processes that support an interdisciplinary approach to discharge planning

1.0 contact hour will be awarded for this continuing nursing education activity. Learners must view at least 80% of the entire video and complete the associated evaluation to receive the contact hours. No partial credit will be awarded. Contact hours will be awarded for this activity until November 2, 2026.
