Extensive readmissions have a wide impact, including clinical and quality of life consequences, financial and regulatory implications, impacts to facility reputation, and a decrease in staff morale and an increase in burnout. Join AAPACN Curriculum Development Specialist, Lauren Stenson, MSN, RN, CNDLTC, QCP, DNS-CT, for an interactive webinar exploring the most common drivers of readmissions—including infections, cardiopulmonary issues, medication mismanagement, and falls—while learning how to use tools to identify risks early and respond effectively. This 60-minute webinar will equip nurse leaders and interdisciplinary team members with practical, evidence-based strategies to prevent avoidable hospital readmissions and improve resident outcomes. Participants will gain actionable approaches to strengthen communication, engage families, optimize chronic disease management, and foster a culture of safety.
After participating in this session, learners will be able to:
- Analyze common clinical, psychosocial, and system-related causes of hospital readmissions
- Differentiate the roles and responsibilities of each interdisciplinary team (IDT) member in identifying and managing at-risk residents
- Recognize evidence-based strategies to reduce avoidable hospital send outs
- Discuss how to develop an action plan to integrate readmission prevention strategies into daily IDT practice and facility-wide QAPI initiatives
1.0 contact hour will be awarded for this continuing nursing education activity. Learners must view at least 80% of the entire video and complete the associated evaluation to receive the contact hours. No partial credit will be awarded. Contact hours will be awarded for this activity until October 16, 2028.
